NHS Digital Weight Management Programme

12 Week Online Programme with 50 – 100 minutes of personalised coaching dependant on support needed.

Need a diagnosis of Hypertension or Diabetes.

Available to NHS staff who do not have Hypertension or Diabetes.

Inclusion Criteria:

  • Aged 18 and over
  • BMI 30 (or 27.5 if from ethnic minority) or over with Diabetes or Hypertension
  • BMI needed within 2 years.

Exclusion Criteria:

  • Moderate or Severe frailty
  • Pregnancy
  • Eating Disorders
  • Bariatric Surgery within 2 years

Isle of Wight Council

Gives access to three separate programmes:
  • Slimming World (face-to-face groups only)
  • Digital Support
  • Exercise Support

Inclusion Criteria: 
  • Resident of the Isle of Wight or registered with an Isle of Wight GP
  • BMI 30 or over
  • BMI 27.5 or over if from ethnic minority
  • Age 18 and over
  • (16- or 17-year-olds can be referred by GP)
Exclusion Criteria:
  • Those with a BMI of less than 30 (BMI <27.5 for ethnic minority population and those with specified long-term conditions)
  • Those under 18 years of age (16 and 17 year olds can attend with GP/Healthcare professional recommendation)
  • Those with severe psychological morbidity and/or eating disorder
  • Pregnant women
  • Those who have accessed the same Service in the previous twelve months.

MAN V FAT Football

MAN V FAT Football is a 14-week weight management program designed specifically for men who love football. It combines small-sided football games with a structured weight management plan.
 
Inclusion Criteria:
 
The population covered by this programme are Isle of Wight residents (or registered with an Isle of Wight GP)
  • Aged 18 or over
  • BMI of 27.5 and over
  • Man V Fat Football is a men-only intervention
Exclusion Criteria:
  • Those with a BMI of less than 27.5
  • Those under 18 years of age
  • Those with severe psychological morbidity and/or eating disorder
  • Those who have accessed the same Service in the previous twelve months.

Tier 2 Non-complex obesity (Behaviour change lifestyle service) via the Wellbeing Service Portsmouth Wellbeing Service – Portsmouth City Council
 
12-week group-based weight management support, with on-going support up to 1 year, via drop-in’s (for all topics weight, smoking, alcohol).
 
Limited 1-1 support for those who meet eligibility criteria for that (shift work which doesn’t allow regular attendance at a group or those with medical condition that impacts attending venue outside home).

  • PO1-PO6 only
  • Adults 18+
  • BMI 30* to 59.9 or *23 BAME
  • BMI 25-35 pregnant

Tier 2 Obesity with comorbidities
 
1-1 support, small group support usually condition specific e.g. PCOS and weight loss etc.
 
Inclusion Criteria:  
  • Age 16+
  • BMI 25+ (inc. pregnancy) with one or more complex/uncontrolled comorbidities
  • BMI>35 or excessive weight gain in pregnancy
  • BMI 50+ no comorbidities
Exclusion Criteria:
 
None – but those uncomplex/no commodities encouraged to access community non-complex lifestyle service.

NHS Talking Therapies Portsmouth

Weight management groups or workshops coming where emotional or mental health is affecting weight loss.
 
Inclusion Criteria: 
  • PO1-PO6
  • Age 16+
  • BMI 25+
Exclusion Criteria:
 
Patients are triaged on entry to service and if deemed unsuitable for core WM offer are supported to access another stream of targeted support within Talking Therapies or referred to more appropriate service.

Southampton City Council

OVIVA UK provides a 12-week weight loss service for Southampton which is a combination of face to face and digital support.
 
Inclusion Criteria: 
 
Participants MUST be those who are at high risk of adverse outcomes due to one or more of the following:
  • Long-term health conditions
  • Disability
  • Their ethnicity (Black African, African-Caribbean, and Asian origin)
  • Are in receipt of a recognised income related benefit.
AND ARE:
  • Are aged 18 years and above.
  • With a BMI ≥30 kg/m2 (adjusted to BMI ≥27.5 kg/m2 for Black, Asian, and Minority Ethnic Groups)
  • Registered with a Southampton City practice and/or be a Southampton City resident.
  • Are deemed suitable by their GP or Health Care Professional to attend the programme.
  • Are willing to commit to a 12-week programme.
  • Be able to participate in face-to-face support in-person (via digital apps and platforms when necessary)
 
Exclusion Criteria:
  • Does not meet the eligibility criteria as defined above
  • Those with severe/moderate frailty as recorded on a frailty register.
  • Those who are pregnant.
  • Those with a diagnosed eating disorder.
  • Those with a significant unmanaged co-morbidity.
  • Those who have had bariatric surgery in the last two years.
