Salaried GP at Swan Medical Group

Position Available:  Salaried GP – 4 to 6 sessions per week

We are seeking an additional salaried GP to join our esteemed team, comprising 14 Partners, 15 salaried GPs, and a robust Nurse team delivering Rapid Access services and Chronic Disease management. With a patient base of 37,000 individuals, this is an exciting opportunity for a motivated GP to contribute to the team.

Location
We operate across five sites in Hampshire, including one in Petersfield, two in Liss, and two in Liphook. As part of our commitment to sustainability and quality care, we are seeking a 4-6 session Salaried GP to work at either of our village surgeries in Liphook, taking on an established patient list.  GP’s will work alongside a supportive team of healthcare professionals including clinical pharmacists, physiotherapists, and a dedicated frailty team.

Appointment Structure

Daily workload is managed by using a triage team. The triage team is responsible for directing patients to the most appropriate clinician or service, ensuring that GP’s only handle cases that specifically require their expertise.  Additionally,   all GP appointments are a minimum of 15 minutes.  This structure is designed to optimise efficiency, reduce the burden on GP’s and improve patient care by matching patients with the appropriate level of clinical support.

Trainee Dispenser/Prescription Clerk

Swan Medical Group has a new opportunity for a Trainee Dispenser/Prescription Clerk to compliment the team at our Dispensary and Medicine Management team in Liss, Hampshire

Hours: The role is available for part-time hours. Morning shifts start at 08:00 and afternoon shift finish at 18:30

Competitive salary: Starting Pay from £11.70, dependent on level of qualifications, experience

Essential: Excellent attention to detail

Duties include:

Medicine Management/Prescription Clerk

  • Regularly collect, check all prescriptions in accordance with the Practice’s Repeat Prescribing protocol, ensuring any queries are passed onto the Doctors as necessary, and deal with any phone queries or voicemail messages left on the prescriptions answering machine.
  • Ensure all prescriptions are up to date on a daily basis and where possible that all prescriptions received that day are processed by the end of that shift, and if not possible within a maximum of 3 working days as per protocol.
  • To deal with any emergency requests promptly and efficiently ensuring these are followed through and signed by the relevant Doctor.
  • To be responsible for distributing prescriptions to GPs for signing where required.
  • Have a sound understanding of basic medical terminology.
  • To accurately log all new deliveries of blank prescription books and monitor stock of allocated GP prescription books using the Prescription Log.
  • To identify when problems arise and alert the Senior/Manager.
  • Ensure all verbal and written requests are acted upon in a professional manner.
  • To maintain efficient administrative and clerical standards to ensure a professional outlook when dealing with patients via phone or face to face.
  • To answer and act upon phone queries and respond to online prescription requests.
  • Regularly check and collect prescription requests from reception and send requests to appropriate GP’s.
  • Check Medicine Management Workflow on EMIS and ensure that all prescriptions are sent to the correct GP. If GP absent, change to alternative GP in accordance with the ‘Buddy System’.
  • Ensure alerts are added to patient record when numerous urgent requests and send template letter if necessary.
  • Update patient’s medical record when correspondence is received from Hospitals or District Nurses these may including hospital letters, discharge summaries.
  • To be responsible for Claimable prescriptions

Trainee Dispenser

  • Take requests for, issue and dispense prescriptions, using Standard Operating Procedures
  • Carry out final check of dispensed medications once authorised to do so.
  • Hand out prescriptions and collect prescription charges or exemption declarations as appropriate.
  • Maintain stock levels of drugs, prescriptions and labels from sources indicated by the Dispensary Senior
  • Receive drug deliveries and check goods received against invoices and claim credits as required for shortages or errors.
  • Ensure the Controlled Drug Register is completed, and stock level checks are carried out as required
  • Comply with safeguarding training requirements as set out by the practice
  • Take prompt and timely action when there are any welfare or safeguarding concerns
  • Form appropriate relationships with those we care for and maintain professional boundaries
  • Work within organisational policies, protocols, and procedures
  • Demonstrate the organisations values and behaviors.

Call Handler/Receptionist

Hours: Part time, to be discussed at interview.

Competitive salary: £11.70 per hour, reviewed after 6 months.

Swan Medical Group is expanding, and we are seeking friendly receptionists to join our team and work across our GP practices in Petersfield, Liphook and Liss in Hampshire.

If you are professional and personable and keen to provide outstanding service to our patient population then this could be the role for you.

You would be joining a friendly, supportive, and established team. Your duties would include working in our telephone hub, front of house receptions desks at all our surgeries, dealing with queries from patients and other administration tasks.

The ideal candidate would be a confident communicator with excellent customer service and administration skills and with previous experience in a public facing role.

Applicants need to be confident with new IT systems, comfortable learning new tasks and be a team player.

At times this can be a challenging role and so it is important that you can always remain calm and professional.

Core Hours: Monday to Friday between the hours of 08:00 – 18:45, part-time shifts would be within these core hours & to be discussed at interview. Some flexibility may be required to cover absences.

Clinical Pharmacist for Swan Medical Group

Hours: Part time – 16 hours per week.

Competitive salary: £42,000.00 to £47,000.00 per year dependent on level of qualification and experience

We have a rare opportunity for a Clinical Pharmacist aspiring to work at this outstanding level. The post holder will support all 5 practices, circa 38,000 patients, by providing expertise in clinical medicines management, working as part of Swan Medical Group’s Medicines Management Team.

The Clinical Pharmacist will provide face to face or remote structured medication reviews, review of patients with polypharmacy including those within residential care homes and those with multiple comorbidities. Additionally, the role will require them to manage long term conditions, medicines on transfer of care and systems for safer prescribing, repeat prescription authorisation, and reauthorisation, as well as acute prescription requests. This is in line with the public health and social care needs of patients in the GP practices.

The post holder will also provide leadership on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within General Practice.

The Post holder will be supported to develop their role and become a non-medical prescriber if that qualification is not already held. Experience and a qualification as an Independent Prescriber may secure a salary at the higher end of our salary range.
