Chronic Obstructive Pulmonary Disease Clinic (COPD)
This chronic lung disease which affects a large percentage of the population can be very debilitating. Our nurse-led COPD clinics aim to help patients to improve their quality of life, optimise their treatment and control symptoms. An annual review provides an opportunity to monitor disease progression, address lifestyle issues and offer support and advice.
All new patients with COPD registering with the practice are invited to attend the COPD clinic and annually thereafter if adequately controlled. We will invite you for your annual review on the month of your birth, but if you think that you might have missed your routine review, have any concerns or feel that you need to update us on your COPD symptoms and management, please complete and submit the form by following the link below. After returning your form it will be reviewed by the team and you may be contacted to discuss your COPD or invited in for a face to face appointment in the COPD clinic. We aim to review submitted forms within 7-10 working days. If you are unable to complete the online review, please contact the surgery for a telephone appointment with one of the respiratory nurses.
