At Swan Medical Group, we believe that good health is about more than treating illness. It’s about helping you to stay well, make positive lifestyle choices, and feel supported in every aspect of your physical, emotional, and social wellbeing.

Our Health & Wellbeing services are designed to help you take a proactive approach to your health. Whether you’re looking to improve your lifestyle, manage a long-term condition, access community support, or simply find the right information, we’re here to help.

We offer support through:

NHS Health Checks

The NHS Health Check is a free health assessment for adults aged 40 to 74 who do not already have certain long-term health conditions. It is designed to help identify your risk of developing conditions such as heart disease, stroke, type 2 diabetes, kidney disease, and dementia.

During your appointment, we will:

  • Check your blood pressure, height, weight, and Body Mass Index (BMI)
  • Take a blood test to measure your cholesterol levels (and blood sugar where appropriate)
  • Ask about your lifestyle, including smoking, alcohol intake, diet, and physical activity
  • Calculate your risk of developing cardiovascular disease over the next 10 years
  • Offer personalised advice and support to help you reduce your risk and improve your long-term health

Following your Health Check, you may be offered further support or referred to services that can help you make positive lifestyle changes, such as smoking cessation, weight management, exercise programmes, or Social Prescribing.

If you’re eligible, you’ll usually be invited every five years. If you think you’re due an NHS Health Check, please contact our reception team or book online.

Social Prescribing

Sometimes improving your health involves more than medical treatment alone.

Many factors such as loneliness, financial worries, housing issues, caring responsibilities, or reduced confidence can have a significant impact on your health and wellbeing.

Our Social Prescribing Link Workers provide personalised support by taking the time to understand what matters most to you. Together, you’ll explore practical ways to improve your wellbeing and connect with local services, community groups, and organisations that can offer ongoing support.

Social Prescribing is available to anyone aged 18 and over who could benefit from additional non-medical support.

Appointments are relaxed, confidential, and centred around your individual goals. Our Link Workers will work alongside you to help you identify what is important to you and support you in making sustainable changes that improve your overall health and wellbeing.

Long-Term Condition Management

We provide ongoing care and support for people living with long-term conditions such as diabetes, asthma, hypertension, and COPD, helping you stay well and manage your health with confidence. You will be invited for your health check usually around the month of your birthday.

If you’re not sure which service is right for you, or you’d like someone to help you explore your options, our Social Prescribing Link Workers can work with you to identify what matters most to you and connect you with the support that’s right for your individual circumstances.
