​What is Medication Monitoring?

At Swan Medical Group, we use a secure online system called MedLink to help safely monitor your medication.

Medication monitoring means checking that your medicines:

  • Are safe for you to continue
  • Are working as expected
  • Do not need adjusting based on your health or test results

This is especially important for medicines that require regular checks, such as blood tests.

How MedLink works

When your medication monitoring is due:

1) You will receive a message

  • By text, email, or via the NHS App
  • This will include a secure link

2) Complete your review online

  • You can use a phone, tablet, or computer
  • Your answers save automatically, so you can come back later
  • A friend or family member can help you if needed

3) Follow any instructions carefully

You may be asked to:

  • Book a blood test
  • Provide a sample
  • Book an appointment

4) We review your results

  • Our clinical team will check your answers and any test results
  • We will contact you if any changes or follow-up are needed

 

⚠️ Important: Booking your physical checks – Completing the online form is only part of your medication review.

If we ask you to:

  • Book a blood test
  • Provide a urine sample
  • Attend for a check (e.g. blood pressure, weight)
  •  

You must still arrange this.

Even if you select “complete without further discussion” – Your review is not complete until all required checks are done.

Failing to complete these checks may mean:

  • Your medication cannot be safely reviewed
  • Your prescription may be delayed or limited

 

Why monitoring is important

Medication monitoring helps us to:

  • Keep your treatment safe and effective
  • Detect any problems early
  • Ensure your prescriptions remain appropriate

It also means:

  • Fewer unnecessary appointments
  • More flexibility to complete reviews at a time that suits you

 

How often will I be contacted?

This depends on your medication.

Some medicines require:

  • Regular blood tests (e.g. every 3, 6 or 12 months)
  • Periodic reviews to ensure safety

We will contact you when your review

 

Is this the same as my annual review?

No.

Medication monitoring is:

  • In addition to any annual long-term health or medication reviews
  • Focused specifically on the safety of your medicines
  • More frequent than medication reviews
