Cardiovascular Disease (CVD) remains one of the leading causes of death and disability in the UK. It encompasses a range of conditions affecting the heart and blood vessels, including heart attacks, strokes, heart failure, and peripheral arterial disease. Many of these conditions are preventable through early identification of risk factors and targeted interventions.

This initiative is designed to tackle heart disease at its roots — by focusing on prevention, early detection, and effective management of risk factors such as high blood pressure, high cholesterol, atrial fibrillation, and lifestyle-related concerns.

What Is the CVD Prevention Contract?

The CVD Prevention Contract is part of NHS England’s long-term plan to reduce the burden of heart and circulatory diseases across the population. The contract provides funding and guidance for GP practices to systematically identify patients at risk of cardiovascular events and offer them timely, evidence-based support.

Through this partnership, Swan Medical Group is delivering proactive, data-driven care that ensures patients receive:

  • Cholesterol and lipid profiling
  • Atrial fibrillation detection using modern diagnostic tools
  • Personalised lifestyle advice including smoking cessation, physical activity support, and dietary counselling
  • Medication reviews and optimisation where appropriate

How You Can Get Involved

If you’re aged 40–74 and have not yet been diagnosed with a cardiovascular condition, you may be eligible for a free NHS Health Check. This simple check can help spot the early signs of CVD and offer advice to help reduce your risk.

Understanding Your NHS Health Check Results & QRISK Score

At Swan Medical Group, we use your NHS Health Check results to assess your risk of developing Cardiovascular Disease (CVD) over the next 10 years. This includes conditions such as angina, heart attacks, strokes, and transient ischaemic attacks (TIAs or ‘mini strokes’).

We calculate your risk using a clinically validated tool called QRISK — which combines information about your lifestyle, weight, blood results, and existing medical conditions to generate a percentage score.

What Does Your QRISK Score Mean?

Your QRISK result gives you a percentage likelihood of developing cardiovascular disease within the next 10 years. For example:

  • A 10% risk means that 10 out of 100 people with a similar profile to you are likely to develop some form of CVD in the next decade.
  • 90 out of 100 people would not.

This includes a spectrum of outcomes, from minor, temporary events (like a TIA) to serious, potentially fatal events (like a stroke or heart attack).

How Is Risk Categorised?

  • Low Risk – Less than 10%
  • Moderate Risk – 10–20%
  • High Risk – Greater than 20%
