Locum GP for maternity cover – 6 sessions per week (Temporary for the duration of maternity cover)

We are seeking a long-term Locum GP to provide 6 sessions per week, primarily based across our Liss surgeries in Hampshire looking after an established patient list.

 We operate across five sites in Hampshire: one in Petersfield, two in Liss, and two in Liphook. You will work as part of a supportive, multidisciplinary team, alongside clinical pharmacists, physiotherapists, and a dedicated frailty team.

 We are a successful GP training practice with two GP trainers, excellent QOF achievement, and a proactive Patient Participation Group. We pride ourselves on being supportive, cohesive, and forward-thinking, with a strong commitment to delivering timely, high-quality care.

About the Role

Due to service growth, we are looking for an additional dedicated and compassionate Social Prescribing Link Worker to join our team. This role supports individuals to take control of their health and wellbeing by focusing on the question, “What matters to me?”

Working closely with patients, you’ll help develop personalised care plans and connect individuals to community services, voluntary organisations, and other support networks. You will also collaborate with a variety of other agencies to strengthen the community resources available for social prescribing.

This role addresses broader determinants of health including mental health challenges, social isolation, poor housing, financial concerns and physical inactivity. It is especially beneficial for individuals managing long-term mental and physical health conditions, or dealing with complex social issues.

Ensuring that patient expectations are managed.

In line with your manager and the Senior Social Prescriber, you will manage your own schedule, dividing time between GP surgeries, home visits, community settings, and administration. While you’ll work autonomously with patients, you’ll be supported by your manager and the wider healthcare team.

  • Support the growth and sustainability of local voluntary and community groups
  • Work collaboratively with GPs, healthcare professionals, and practice staff
  • Attend multidisciplinary team (MDT) meetings and maintain patient records
  • Contribute to the ongoing mapping and development of local support services

Swan Medical Group has a rare opportunity for a Practice Nurse to cover a period of maternity leave. The role holder will work across our practices in Petersfield, Liphook & Liss in Hampshire.

Hours: Monday to Friday

Part Time/Full Time Temporary Maternity Cover

Pay: £15.00 to £17.85 per hours, dependent on level of qualifications, experience & specialist skills

We are a supportive and forward-thinking practice with an excellent clinical team. We are a training practice for doctors and are supportive of personal development for all staff.

The successful candidate must have a current NMC registration.

The role involves treatment room duties including dressings, child immunisations, women’s health etc. There could potentially be the opportunity to specialise in long term conditions including Asthma, COPD and Hypertension.

About the Role

The Data & Digital Administrator will support the practice in delivering high-quality, data-driven, patient-focused care. The role involves managing digital health tools, improving data quality, supporting recall and optimisation processes, and helping patients and staff make the best use of digital technologies.

This position is central to enabling efficient workflows, supporting clinical decision-making, and ensuring the practice meets contractual, statutory, and quality improvement requirements.

Key Responsibilities

Information & Data Quality

Manage, analyse and audit clinical data to ensure accuracy, quality and compliance with GDPR.

Produce reports and searches to support clinical audit, operational planning and population health management.

Develop and maintain clinical templates to improve data capture and coding.

Promote best practice in data management and provide training to staff.

Present data clearly to support decision-making and monitor the impact of service improvements.

Performance & Target Management

Monitor and report on QOF, IIF and Enhanced Services performance, working closely with clinical and management teams.

Maintain accurate disease registers and robust patient recall and screening systems.

Coordinate administrative requirements for Enhanced Services, including data submissions, CQRS reporting and performance tracking.

Maintain financial tracking of Enhanced Services income and ensure all payments are received.

Support practice and PCN initiatives through participation in working groups and project teams.

Digital & Information Technology

Support the maintenance of practice websites, intranet, patient information displays and communication channels, including social media.

Develop and maintain strong knowledge of practice digital systems including EMIS, CQRS, Ardens, Microsoft Office and related platforms.

Analyse and present data from multiple systems (e.g. telephone, eConsult and appointment systems) to support demand and capacity planning.

Use advanced Excel skills, including pivot tables, to present meaningful insights.

Support staff to optimise use of digital systems and new technologies.

Administrative Support to Clinical Teams

Provide reporting, data searches and administrative support to clinical managers and teams.

Support rota management, appointment setup and room utilisation.

Assist with vaccination programmes, NHS Health Checks and patient recall processes, including data reporting and financial reconciliation.

Coordinate patient communications, invitations and activity monitoring across clinical projects.
