We are seeking a Summer Locum GP to provide 6 sessions per week, primarily based in our Petersfield surgery in Hampshire, looking after an established patient list.

We operate across five sites in Hampshire: one in Petersfield, two in Liss, and two in Liphook. You will work as part of a supportive, multidisciplinary team, alongside clinical pharmacists, physiotherapists, and a dedicated frailty team.

We are a successful GP training practice with two GP trainers, excellent QOF achievement, and a proactive Patient Participation Group. We pride ourselves on being supportive, cohesive, and forward-thinking, with a strong commitment to delivering timely, high-quality care.

We are seeking an additional salaried GP to join our esteemed team, comprising 11 Partners, 18 salaried GPs, and a robust Nurse team delivering Rapid Access services and Chronic Disease management. With a patient base of 37,000 individuals, this is an exciting opportunity for a motivated GP to contribute to the team.

Location

We operate across five sites in Hampshire, including one in Petersfield, two in Liss, and two in Liphook. As part of our commitment to sustainability and quality care, we are seeking a 4 session Salaried GP to work across our village surgeries in Liss & Swan, Surgery in Petersfield, taking on an established patient list.  GP’s will work alongside a supportive team of healthcare professionals including clinical pharmacists, physiotherapists, and a dedicated frailty team.

Job Purpose

To provide a flexible and efficient mix of administrative and secretarial support across Swan Medical Group, supporting patient care and smooth practice operations. The post holder will work closely with clinical and non-clinical staff, undertaking a combination of routine admin tasks, document management, correspondence handling, and secretarial duties such as processing referrals, medical reports, and correspondence from clinicians.

Key Responsibilities

Administrative Duties

  • Answer incoming telephone calls in a professional manner and respond to queries or direct appropriately
  • Manage patient registrations, deductions, and demographic updates
  • Scan, code, and file documents into the clinical system (EMIS Web) accurately and in a timely manner
  • Process tasks in line with clinical workflow protocols (e.g., lab results, discharge summaries, hospital letters)
  • Maintain accurate records of tasks undertaken via internal trackers or logs
  • Respond to patient emails and internal communications via NHS Mail and Accurx
  • Assist with administrative elements of safeguarding, recalls, long-term condition reviews, and clinical searches
  • Cover reception duties when required
  • Maintain confidentiality and adhere to information governance standards at all times

Secretarial Duties

  • Process referrals via the NHS e-Referral System (ERS), ensuring referral quality and completeness
  • Liaise with patients regarding referrals, appointments, and letters
  • Track and process non-NHS medical reports (e.g., insurance, solicitor, DWP, capacity forms), following payment and workflow protocols
  • Prepare and format medical reports or letters as requested by GPs
  • Manage “choose and book” queries and ensure follow-ups are actioned
  • Support GPs and other clinicians with documentation and admin linked to their clinics

About the Role

Due to service growth, we are looking for an additional dedicated and compassionate Social Prescribing Link Worker to join our team. This role supports individuals to take control of their health and wellbeing by focusing on the question, “What matters to me?”

Working closely with patients, you’ll help develop personalised care plans and connect individuals to community services, voluntary organisations, and other support networks. You will also collaborate with a variety of other agencies to strengthen the community resources available for social prescribing.

This role addresses broader determinants of health including mental health challenges, social isolation, poor housing, financial concerns and physical inactivity. It is especially beneficial for individuals managing long-term mental and physical health conditions, or dealing with complex social issues.

Ensuring that patient expectations are managed.

In line with your manager and the Senior Social Prescriber, you will manage your own schedule, dividing time between GP surgeries, home visits, community settings, and administration. While you’ll work autonomously with patients, you’ll be supported by your manager and the wider healthcare team.
