We are seeking a Long-Term list holding Locum GP to provide 4-8 sessions per week, primarily based in our Liphook surgeries in Hampshire, looking after an established patient list.
We operate across five sites in Hampshire: one in Petersfield, two in Liss, and two in Liphook. You will work as part of a supportive, multidisciplinary team, alongside clinical pharmacists, physiotherapists, and a dedicated frailty team.
We are a successful GP training practice with two GP trainers, excellent QOF achievement, and a proactive Patient Participation Group. We pride ourselves on being supportive, cohesive, and forward-thinking, with a strong commitment to delivering timely, high-quality care.

About Swan Medical Group:
We are seeking an additional salaried GP to join our esteemed team, comprising 12 Partners, 17 salaried GPs, and a robust Nurse team delivering Rapid Access services and Chronic Disease management. With a patient base of 37,000 individuals, this is an exciting opportunity for a motivated GP to contribute to the team.
Location:
We operate across five sites in Hampshire, including one in Petersfield, two in Liss, and two in Liphook. As part of our commitment to sustainability and quality care, we are seeking a 4 to 8 session Salaried GP to work across our two village surgeries in Liphook, taking on an established patient list. GP’s will work alongside a supportive team of healthcare professionals including clinical pharmacists, physiotherapists, and a dedicated frailty team.

Appointment Structure:
Daily workload is managed by using a triage team. The triage team is responsible for directing patients to the most appropriate clinician or service, ensuring that GP’s only handle cases that specifically require their expertise. Additionally, all GP appointments are a minimum of 15 minutes. This structure is designed to optimise efficiency, reduce the burden on GP’s and improve patient care by matching patients with the appropriate level of clinical support.

Acute Care Nurse Practitioner

We have a new and exciting opportunity for an experienced clinician to join our established team of Nurse Practitioners and Paramedics. The post holder will work across all sites. The team provide acute minor illness care to patients and Home Visiting Service, aiming to prevent A&E visits and hospital admissions.
We are supportive and forward-thinking practice with an excellent clinical team. We are also a training practice for doctors and are supportive of personal development & ongoing training for all staff. We aim to provide our own succession planning.
Our Practitioners are supported to work independently, autonomously delivering care and treatment to patients within the clinical setting, having gained a wide range of knowledge and clinical skills in Primary Care. These skills will enable the post holder to provide the highest standard of care.
The successful candidate will undertake face to face clinical assessment and treatment of patients for minor illness, including undertaking telephone consultations and triage.
Applications are welcomed from experienced clinicians who have organisational skills and strong time management skills to ensure patients are seen in a timely way and that clinic run effectively, without compromising patient care or safety.

About the Role
- Accurate data entry and maintaining electronic records, spreadsheets and trackers
- Supporting data collection, reporting, audits and compliance activities
- Completing regular checks, testing and recording results

- Providing basic IT support and liaising with our external IT provider
