This role requires flexibility, resilience, and a willingness to step beyond your comfort zone; however, it is a highly rewarding opportunity to make a meaningful difference in people’s lives.

- Can effectively build relationships with patients, colleagues, and local organisations
- Is confident working with people from diverse cultural and social backgrounds
- Thrives in challenging environments and is committed to inclusive, person-centred care
You will also need to be driven and able to work independently whist attending multidisciplinary team meetings. A sound understanding of local health, social care, and voluntary sector services is beneficial.
Key Responsibilities
- Build trusting, supportive relationships with patients
- Develop and implement personalised care and support plans
- Signpost and connect individuals to community-based resources
- Support the growth and sustainability of local voluntary and community groups
